In order to fill your request for materials, please provide us with the following information about you:

  • Last name, first name, middle initial
  • Campus ID number
  • Mailing Address
  • Email Address
    (An email address is needed if you wish to receive articles electronically.)
  • Phone (day and evening)
  • Center where attending class
  • Course (e.g. MSA 600, EDU 660)
  • Status (graduate, undergraduate, faculty or staff)
  • Not needed after date
    (This is the last possible date we can send items that might be checked out or missing from our shelves; all materials on the shelf will be sent within 48 hours of the request.)
