Instructions for filling out Coursepack/Copyright Request Forms
Note: The Requestor Information Form is needed each time you teach a class. All other forms only need completed if you are creating an entirely new coursepack or revising an existing one.
These forms are available on the OCLS website. Just click on "Coursepack/Copyright Support " under Faculty Support on the homepage. The forms can be submitted directly to the Coursepack/Copyright Office by clicking on the "Submit" button located at the bottom of each form.
Your coursepack material will be scanned and placed on your Blackboard website.

For all requests
Articles:
For an article, fill out the Form to Request Copyright Permission - Articles. Complete one of these forms for each periodical item in your coursepack. Information needed is the
- title of the journal
- title of the article
- complete author(s) name(s)
Include complete first and last names and middle initial, if any.
- volume
- date
- page numbers
If page numbers are discontinuous, please state the page numbers exactly. Often publishers charge by the page and if we state pages as continuous when actually there are several ads or other articles interspersed, we are charged for several pages that are not being used.
Include the following information only if applicable
- title of the anthology and the year
Note that in books that are collections of readings, the articles may have been copied with permission from other sources. The book from which you are copying that material usually includes the source information at length either in the front or the back of the book or at the bottom of the first page of each reading. That is the information that must be supplied for this request.
- Complete web site name and address
Note that even if a journal's articles are included as full text online, this does not mean that multiple copies may be made of the article without obtaining copyright permission. Many sites require permission and some even charge copyright fees for permission.

Book excerpts:
For a book excerpt, fill out the Form to Request Copyright Permission - Book Excerpts. Complete one of these forms for each book excerpt in your course pack. Include the
- title of the book or document
- author(s) name (s) (first and last names, including middle initial if any)
- date
- ISBN (if available)
- publisher
- page numbers
For edited books of readings by different authors, provide the
- title of the book/anthology
- editor(s) name (s) (first and last names, including middle initial if any)
- author(s) of the reading (first and last names, including the middle initial, if any)
- title of the reading
- source of the reading, if published from a primary source or used with permission
Please note that in books that are collections of readings, such as cases, poems, stories, etc., the material may have been copied with permission from other sources. Cartoons/Illustrations:
For a comic strip/illustration, fill out the Form to Request Copyright Permission - Cartoons/Illustrations. Complete one of these forms for each comic strip/illustration item in your course pack. Provide the
- title of the strip
- illustrator
Include complete first and last names and middle initial, if any
- syndicate
The syndicate is usually included in tiny script along the side of the strip
- date the strip was published
The date (month and day) is usually included somewhere in the last clip of the strip; if you know the year or approximate year, please provide that.
Please note you must supply the master of the cartoon strip and that it must include the title located above the strip. The syndicates require that the title of the strip be included in the copies made as one of the conditions for permission to copy.

Sheet Music:
For sheet music, fill out the Form to Request Copyright Permission - Sheet Music and complete one of these forms for each song or composition you are requesting to include in your coursepack. Information needed is the
- title of composition
- composer
- date the composition was published
- publisher of the composition
- title of the songbook or anthology in which the composition is included (if any)
- date the songbook/anthology was published
- compiler/editor of songbook/anthology
- page numbers
- publisher of the songbook/anthology
Please note that you must supply a copy of the song or sheet music you are requesting to include in your coursepack. If you are copying the song from a book of sheet music composed by different authors, include the title and the copyright pages of the book.

Law Cases:
For a law case, fill out the Form to Request Copyright Permission - Law Cases. Complete one of these forms for each law case you are requesting to include in your coursepack. Include the
- title of the case
This is generally one party vs another.
- citation
This includes the volume and page number of the reporter in which the case is published, e.g. 224 F 2d 448
Note that although the case may be in the public domain, permission must be obtained to use the format and notes of the particular reporter or database in which the case is published.
For any material you are requesting
Please provide as much information as possible to enable your requests to be processed as efficiently as possible.
As noted previously, if you wish to use sheet music or cartoon strips or illustrations, you must provide the masters for these. If Central Michigan University Libraries has access to the books and periodicals from which you are requesting material, the masters for the copies can be obtained here; masters can also be obtained through interlibrary loan. If you prefer to supply your own masters for articles and book excerpts, copies should be single-sided on 8 ½ x 11 inch paper; do not reduce the size of the print.
Please submit your requests by e-mail. The forms can be submitted directly to the Coursepack/Copyright Office by clicking on the "Submit" button located at the bottom of each form.
