Instructions for filling out Copyright Request Forms for Multimedia
If you are creating an entirely new project or revising a former one, you need to complete the copyright forms. The forms can be submitted directly to the Coursepack/Copyright Office by clicking on the "Submit Request " button located at the bottom of each form. If you do not have access to a computer, call the Coursepack/Copyright Office at 800-274-3838 or 989-774-3828 and ask that the forms be sent to you.
Complete one Requestor Information Form per class for which you are using the multimedia project. Information needed on the Requestor Information Form is your name, address, phone number, e-mail address, course number and title, course locations (centers), and the beginning and ending dates of the time period for which you plan to use the multimedia project.
Note: If you are reusing material from a previous multimedia project, simply fill out the Requestor Information Form. You do not need to fill out any other forms.

Music:
For portions of songs or other musical compositions you wish to include in a video or other project, you must fill out the Form to Request Copyright Permission - Cuts from CD's/LP's.
On the form, check whether you are requesting synchronization or mechanical licensing rights.
- Example of synchronization rights:
You want to include Copland's Saturday Night Waltz as background in a video you are producing; the music must be synchronized with the video's script; you are asking for synchronization rights.
- Example of mechanical licensing rights:
You want to include a cut from The Girl from Ipanema on a tape or CD as an example of world music for each member of your class; you are asking for mechanical licensing rights.
Other information you need to provide:
- title of the composition
- Composers of the composition [include complete first and last names and middle initial, if any]

length [in minutes and seconds] of the excerpt to be copied

length of the original recording [also provide in minutes and seconds to determine what percentage of the composition you wish to copy]

time sequence in which the excerpt occurs [state the time in minutes and seconds from when the excerpt you wish to copy starts to where it ends - e.g., 2 minutes 10 seconds to 3 minutes 45 seconds(2:10-3:45)]

purpose of creating the video or project
- number of copies of the video or project you plan to create
- state whether the copies of the project [e.g. tape/CD/video, etc.] are to be permanently distributed or retained by you

if distributed, state the mode of distribution [be specific, e.g. via Blackboard, ITV, etc.]
- mode of instruction [state whether instruction is to be face-to-face or online; if online, what type. (e.g., ITV, Blackboard, etc.)]

length of time to be covered by the contract [state how long you plan to use the project or video without change (e.g., one year, two years, etc.)]
If you are requesting mechanical licensing rights, also provide
- format (e.g., tapes, CD's, etc.) to be used

Motion pictures and TV Programs:
Clips:
If you are making a multimedia project or video and plan to include clips from motion pictures or documentaries, fill out the Form to Request Copyright Permission - Clips from Videos/Films. If you want to incorporate clips from TV programs, fill out the Form to Request Copyright Permission - Clips from TV Programs. Complete one of these for each motion picture, documentary or TV program from which you want to use clips.
The following information is needed:
- title of the movie or television show
- movie or TV studio
- if the clip is from a TV program, the title of the episode and/or the episode's number
- length of the clip [in minutes and seconds]
- length of the film or television program [in minutes and seconds]
- time sequence of the clip within the film or program [e.g., the first 3 minutes or the section, 30 minutes, 45 seconds to 37 minutes 48 seconds (30:45 - 37:48)]
- description of the scene in the clip
- intended use of clip in the project
- title of project
- purpose of creating the project
- synopsis of project
- script pages [just that script pertaining to the clip]
- number of copies of project
- state whether copies of the project will be distributed or retained by instructor
- intended mode of distribution [be specific, e.g., blackboard, ITV, etc.]
- countries of distribution [worldwide, intrastate, local, etc.]
- mode of instruction [state whether instruction is to be face-to-face or online; if online, what type, (e.g., ITV, blackboard, etc.)]
- length of time the contract needs to cover [state how long you plan to use the project without change, e.g., one year, two years, etc.]

Stills:
If you are making a multimedia project or video and plan to include stills from motion pictures or documentaries, fill out the Form to Request Copyright Permission - Stills from Videos/Films. For stills from television shows, fill out the Form to Request Copyright Permission - Stills from TV Programs. Complete one of these forms for each motion picture or documentary or television program from which you want to use stills.
For stills, provide the same information listed under clips, with the following two exceptions:
- Instead of the script, just provide the text or caption meant to accompany each still.
- In lieu of describing the scene in the still, you may opt to attach a copy of the still.

Photographs/Illustrations:
If you are making a multimedia project or video and plan to include photographs or illustrations, fill out the Form to Request Copyright Permission to Digitize Images. Complete one of these for each image you want to use.
Sometimes clearance for images can be quite simple, sometimes very complex, depending on how many generations or layers the image is from the original work of art.
If you have created a digital or analog image of an original work of art, such as architecture, a painting, or piece of sculpture, and the work of art is not in the public domain, we need to obtain the permission of the original architect, painter, or sculptor to use the image.
Provide
- the title of the original work of art
- the name of the artist
- a copy of the image you intend to use
If the image you want to incorporate is a reproduction of an original visual image, such as a print, 35 mm slide or color transparency, we will need to obtain permission of the creator of the slide, print or transparency, as well as permission for the original visual image, the underlying work.
In this case, provide
- the title of the original work of art
- the name of the artist
- a copy of the image you intend to use
- the type of reproduction [generally, reproductions are photographic, including prints, 35 mm slides and color transparencies]
- the name of the creator of the reproduction [e.g., the photographer]
If the image you want to use is a published reproduction, such as a plate of a work of art included in a book or catalog or a digital image in a CD-ROM or at a web site, the publisher of the book, CD-Rom or web site must be contacted for permission, as well as the creators of the plate or digital image and the artist who created the underlying work.
In this situation, provide
- the title of the original work of art
- the name of the artist
- a copy of the image you intend to use
- the type of reproduction [generally, reproductions are photographic, including prints, 35 mm slides and color transparencies]
- the name of the creator of the reproduction [ e.g., the photographer]
- the title of the book or CD-ROM [if the image is online, provide the web site address instead]
- the editor(s) or compiler(s)
- the publisher
- the date published
- the page(s)
If the image is taken from a copy of a published reproduction, such as a 35 mm slide taken of a plate in a published catalog, then the photographer who made the 35 mm slide must be contacted as well as the publisher of the catalog, the maker of the plate and the artist of the underlying work.
In this case, provide
- the title of the original work of art
- the name of the artist
- a copy of the image you intend to use
- the type of reproduction [generally, reproductions are photographic, including prints, 35 mm slides and color transparencies]
- the name of the creator or photographer
- the title of the book or CD-ROM [if the image is online, provide the web site address instead]
- the editor(s) or compiler(s)
- the publisher
- the date published
- the page(s)
- the creator of the copy of the published reproduction [e.g., the maker of the 35 mm slide in the example given]
Whether the image is of an original or many generations removed from the original, you must provide the following information as well:
- title of project
- purpose of creating the project
- intended use of image in project
- number of copies of project needed
- whether the copies of the project are to be distributed to the students or retained by the instructor
- if the project is to be distributed, the mode of distribution [e.g., blackboard, ITV, etc.]
- mode of instruction [state whether instruction is to be face-to-face or online; if online, what type, (e.g., ITV, blackboard, etc.)]
- length of time to be covered by the contract [state how long you plan to use the project without change, e.g., one year, two years, etc.]
